It's a terrible s/l but really is best I can do with how fast she dictates. Always, always, indistinguishable and run together this way.

Vital Signs:  Blood pressure 168/102, pulse 95. Respiratory rate is 16, oxygen saturation 100% on room air, *s/l "wushterpizhil"*, and temperature is 98.4.

All I can think of is weight something... - sm

Those docs that think it's a speed race to dictate this stuff make me nuts. They need to be forced to listen to someone else doing that and see if THEY can figure out what the heck they're saying.

Got another by same physician and this time "normal" is very clear at end of that phrase and not sure if I hear the w sound at beginning this time.

Vital signs: Blood pressure 157/90. He was initially tachycardic on arrival at 122, respiratory rate of 18, oxygen saturation 98% on room air *s/l "turshin normal" or "witurshin normal"*. Temperature is 97°F.

Would it be normal for a doc to say after the saturation "which is interpreted as normal" every time whether 100% or in the 90s or whatever? It always sounds pretty much the same with this doc no matter what the numbers are. Thanks.

Maybe something like with exertion normal, as in oxygen saturation at rest and with exertion? Again, just a guess. I really hope you figure this one out. It would be driving me nuts, too!

I'm doing another one today. This one sounds more like, "which appears normal," or "She appears normal." However, I keep listening over and over and trying to hear something like you're saing hat makes more sense there, and I think if anything, she's probably saying, "Chest excursion normal." I'm torn between going ahead and putting it or leaving a blank again. However, I'm afraid if I leave a blank, they're probably going to just fill in something like "which appears normal," which I don't think is right (I think I even got a sample of hers one time tha had that, but I still don't think it's right. I think it does have to do with excursion. Have almost talked myslf into putting it.

Different docs comment on different things. What kind of doc is it? What is the pt being seen for?

Today's patient is abdominal pain, but it wouldn't matter if it was headache or anything else. She's always going to say this after the saturation.

...if we work for the same company and are talking about the same doctor (if yours is a female). It does sound very much like she's simply saying something like that, but I'm still not convinced that it's not something along the lines of "Chest excursion normal."

I have a new doctor who gave his vital signs as follows: "90/72, 125, 18, 99.6, and 92%".  The first (bp) and last two (temp and pulse ox) are self explanatory.  I don't know what the 125 and 18 mean, though.  I'm sure this is one I'll kick myself over. Thanks in advance for your help. ...

Can anyone help with any tips on numbers? I work in speech recognition as most of us do now and I am having the hardest time with numbers. The account I am on has high ESL also, but even without that, I can't seem to get values correct 100% of the time to save my life. Examples are hearing a 0 instead of an 8 such as 130 instead of 138 or hearing a 2 instead do a 3 or even in platelets thinking I am hearing a value of 300,000 versus 200,000. If it is a profile that doesn't make sense ...

I seem to be spending an awful lot of time in VR documents, so I am confused as to why I am getting paid less for them.  This stupid VR system types whatever it thinks it might be hearing and sometimes doesn't type anything at all.  I hate this VR.  I am up to 1800 lines but usually do 2200-2500 a day on straight transcription.  I don't know how I am going to feed my children.  I am a single mom and the head of household.  Believe me the child support isn& ...
