Pnt suffered a CVA, doc says . . .

He has had the recent surgery, certainly he could have loose plaque and grim-a-tous plaque. 

 

Can't find a reference so please help if you can.

Lately, I have been getting dinged for not putting hyphens in mild/moderate to moderate/severe plaque or moderate to severe htn or mild to moderate anything.  I don't understand why we would need hyphens.  In that case, shouldn't "anything to anything" need hyphens? ...
