Living in fear of critical error. - Newatthis

Posted: Jan 24, 2010

I've got this terror of critical errors.  I shudder every time I check my feedback, if I've left a marker (sometimes I've noticed they'll check the entire report, not just the marker.  Guess it depends on the MME).  What happens if you get a critical error?  Are you back on QA for awhile?  Do you get nasty e-mails?  Do you get canned?  40 lashes?

Personally, I read over them, learn from the mistake and get on with my job and try not to repeat the same mistake. I don't obsess over it - everyone makes mistakes occasionally, even the very best MTs. We are, after all, human.

I got one - shipwrecked

I got a critical error for an abbreviation in the assessment. Nothing happened at all. No nasty email, no performance management, nothing.

Got a critical a while back. CCM sent me an e.mail advising me of my mistake. The tone of her e.mail was "I've go to do this; its my job." I replied, told her how sorry I was and would do my best for it not to happen again. That was the end of it. I did my part and took responsibility. Think that is basically what they wanted. After all have been doing this >20 years and am human. Am going to make a mistake once in a while :) Don't worry about it!!

Thanks for the comments. - Newatthis

Maybe when I've got a few years under my belt I'll feel a little less stressed out about it. I try to strike the proper balance between making a living i.e. not spending three times as much time on a report as necessary, and not being obsessive.

When an MT makes a critical error they will usually send you an email letting you know of the error so hopefully you can prevent it from happening again. If an MT makes repeated critical errors they would most likely put that person on "Performance Management" which is sort of like a "probation period" whereas you have time to improve your work performance - usually several pay periods. After that, if you fail to improve then you might be let go, but otherwise critical errors happen occasionally to nearly every MT. I remember being a "Newbie" and being scared of making mistakes but after many years of doing this work I have sort of "mellowed out." LOL.

I know the feeling. Each time I bring up my reports that have gone through QA I get butterflies in my stomach and I have been doing this for 33 years!!! Please do not think I am bragging, but I have never gotten one. I think that is why I live in fear of my QA. You make some good points about not being too obsessive and spending too much time on a report. I am really a perfectionist about my MT and sometimes I find myself spending too much time researching or listening over and over to get a word. Unfortunately, my perfectionistic qualities stop with my MT. Anyway, let's both try and take it a littl easier next time we open up that DocQmanage, okay? As you are just starting out in your career and I am getting ready to retire, I hope you enjoy this profession as much as I have all these years. I can honestly say I have loved every minute of it.

Eagle Eye, Ellie, THANK YOU! nm - Newatthis

ERROR ON CC - ELLIE

[ In Reply To ..]
They considered the cc error critical because of what, HIPPA? When was this?

Living in fear - oops

[ In Reply To ..]
Get used to living in fear. It goes along with this job at MQ. Or be bold and just dare them to actually fire you. Don't think they will be that quick to do that. They need the people thay have and it is near impossible to do everything perfectly.

This too shall pass. At least that has been my experience. Today they threaten you, next week it is something else.

Keep it in perspective. This is not a friendly, warm workplace. Work hard while you are on the clock, then concentrate on what makes you happy in life and don't think about work.

What is considered a critical error at MQ, besides... - sm

[ In Reply To ..]
mistakes in medication, spelling, unit, dosage, times, etc...
wrong spelling of Dr.'s and patient's name
wrong cc sending

what else?

Thanks!

I got 4 in one day, and I'm still here. Of course, 3 of them - mt

were wrong, and the other was unintelligible. I challenged them and got no response.

Living In Fear
Dec 04, 2013

Just got e-mail stating I did NOT pass my audit.  i got 99.4%.  i have been with this company for over 15 years.  Lately, I stress out every day over this job.  I am sitting here waiting for a phone call from my TSM to discuss this.  I am quite worried.  Living paycheck to paycheck now.  Cannot afford to take another pay cut or lose my job.  If they do put you on 100% do you ever get taken off? I have slowed down and really tried to be more careful/cautio ...

Got A Critical Error For "omitting Dictation Of A Critical Nature."
Jan 04, 2010

The dictator said "... will get cardiac enzymes and will get a carotid Doppler in the morning...", or something similar, I have deleted the report.  I left out the second will.  This is not a verbatim account.  Critical error for omitted dictation pertains to lab results, normal values, etc.  I didn't think the word will meets that criteria.  I also had Tylenol No. 3 changed to Tylenol #3.  The drug lists still show it as No. 3.  ...

Apr 19, 2014

I'm supposed to provide feedback. Okay, fine, here it is: It was a typo. I sure learned a lot from your snarky auto message. Why do you have to keep making people feel bad for making a freaking typing error? Whoever is running the show at Nuance really needs to seek help. All you (they) can do is cut our wages and find fault with our work? It must be stupendously reassuring to be you and be so perfect. ...

Critical Error On Job Feedback
Nov 13, 2013

I received a notice of critical error that was in red and would not allow me to read the other feedback reports where I left blanks.  Since we have been warned so much about critical errors and how much weight they have, I was afraid to open this report.  The critical error was :  Alert an oriented (but what I should have typed was "alert and oriented."  I forgot the "d" on "and." I can think of so many other things that would have been a crtical error, like leaving out a di ...

Critical Error...my Backside!
Mar 18, 2014

So...I was just dinged for a critical error.  Supposedly, I typed an incorrect medication.  I specifically remembered this dictation and remembered the medication I typed, which was NOT the medication marked in the critical error.  SO - I went into my statistics for the day, pulled up the detail for that particular job and there it was....the CORRECT medication.  Somehow, the correct medication was taken out and the incorrect one was put back in.     You better be ...

Since When Is Misspelling A Doc Name A Major Critical Error????
Feb 14, 2013

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it.  So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? WTH. Seems like they are grasping at straws now. What's the deal? We are to be mind readers now and get these names exact every time even when they don't spell, we can't find, etc. etc. Nuance just makes itself look ridiculous ...

So I Finally Got The Dreaded CRITICAL LAB VALUE ERROR
Mar 04, 2015

So 3 points off...short report....and, course failed, 88%......I'm doomed for the next 2 weeks.  Probably now way to get up over 99% at this point.   Anyway, so I went back to check on it (I usually don't bother), I was going to try to relisten (still can't for whatever reason)...but what I DID see was that the report was sent to the client with the apparently incorrect value of 0.5 still on the report.   So they find the error but don't correct it?   ...

Audits - How Can You Get A "critical" Error And Still Get 99.3% On An Audit, Thereby
Sep 11, 2011

barely scraping by and passing according to MQ?  I just received an audit that had a score of 99.3%.   I'm told I passed the audit but got a critical error (left out a word) so she will need to do another audit next week.  Fine. But how can I get a critical error (in their eyes anyway) and still "pass" the audit?  More importantly, does anyone know if my pay is going to be docked because of this critical error, or do you just have to get 99% period.   TIA ...

It May Be People Like Turning Worm Who Caused MQ To React And Lay Off For Fear Sm
Apr 04, 2013

of a possible class action lawsuit. If you started taking things to the Labor Board you can bet MQ will be forced to let go of people. I'm not saying MM is not to blame because I DO believe they are sending 80% of the work to India for cheap labor and to avoid excessive taxes. They have a right to do that. MM provided work to American workers to provide some with jobs, not to make a profit. It's expensive to employ American MTs thanks to the US Govt, but that's for another day.&nb ...

Cant Seem To Make A Living
