I worked for MDs in an office. One of the MDs would jump around from patient to patient in his dictation because he'd dictate as he was physically seeing the patients that way. That is, he'd see Mr. Smith and dictate something, but while Mr. Smith was getting blood work, he'd see Ms. Jones, and he'd dictate something on Ms. Jones. Then while Ms. Jones was changing, he'd dictate some more on Mr. Smith. Now and then he'd forget to say the name, but since the sex was different I could tell. But how many times did he do it between two male patients or two female patients? In that case, the note could end up in the wrong chart. And whose fault would that be? That's not the type of error a dictator would ever recognize making. |
