I'm one of those high earners, and I'd say a big portion of it is luck - the right account/options of OT sometimes makes all the difference in the world. But a big portion of it is noticing the patterns.  Does Dr. A use the same phrase repeatedly?  Put it in your expander.  Does work dry up on the last day of your week?  Work a little extra the other days, so you'll have a cushion.  Can you change your shift to when there's more work, easier docs, etc.? Another big portion of it is attitude.  Going to work gung-ho, determined to beat them at their game today and come out the winner.
