Now I've heard it all. Patient is 53 yo female, lives alone in a mobile home. Sprained both ankles, unable to bear weight on either ankle. No family/friends to take care of her as they work or in school. Asks if she can go to an assisted living until she is able to bear weight. Doctor tells her to..... get this....  just crawl around her mobile home if she needs to get around.

Fifteen years of doing this and I'm so pissed off right now that I feel like writing him a letter.

I couldn't believe what I was hearing. I've had a few docs that I thought shouldn't be practicing but this one absolutely stunned me. If he had been my doctor and told me to crawl around I would have seen that his license got suspended. Poor woman. And you know people just go about their business and listen to these kooks because they are "doctors".

But even if the poor woman could "drag" herself around her mobile home, if she can't bear weight, how is she supposed to use the toilet or get into the bathtub or even pull herself up to the stove to cook or even heat something up in the microwave??? Or even get into bed at night? All of those ADLs require at least some weightbearing.

That's probably one of the cruelest things I've ever heard of a clinician dictating. Yet, sadly, I'm not entirely shocked.

In fact, if people could HEAR what and how their docs dictate, they would not be too happy.

I think our job has become a bit cruel actually (not that all docs know this) because we are asked to transcribe at 98% or better accuracy from docs who speak worse than they write ! and then to do so on PRODUCTION based pay with rules that change from client to client, service to service, etc., ad nauseam.

Acquaintance falls, badly breaks ankle. This person is a hard worker but poor and at the time, out of work and thus indigent. Goes to closest hospital that treats the poor, is sent home with instructions to go to another such hospital 70 miles away, which was the closest orthopedist to set the break (which required surgery).

The person with the broken ankle could not drive and was in terrible pain. Medicaid will take people to a hospital in an ambulance, and to this day I cannot figure out why some such arrangement could not have been made.

In the meantime, the patient can't do yard work and too proud to allow anyone to help. Big mistake. Code Enforcement comes along and slaps on a $200 fine for tall grass, no excuses allowed, then threatens arrest if the person cannot get into the courthouse that has no wheelchair access.

That was straightened out and the fine dropped, but they don't usually do that.

I was in Arizona on a trip and my left ankle started to swell. No biggie, I thought, but by day three, it was hanging over the sides of my shoe, almost touching the ground. A paramedic friend saw it and said I had to go to the ER NOW - I had red streaks up the back of my leg. I said I will when I get home. 20 hours later, I get home and go to the ER. By now, ankle and foot so swollen it was struck straight. I have no insurance.

They assess me and take me straight in. Doc comes in and says we think its gout. Stick this giant needle into three spots in my ankle joint. No fluid, no crystals. Hmm. Blood work comes out clean - white cells are completely normal. No bug bites, no scratches/scrapes. As I'm lying on the table, it starts in my right ankle and rules out anything they can come up with other than some sort of blood infection. Won't call infectious disease because I have no insurance.

Start 3 different IV antibiotics and the next morning, discharge me (still in ER). They come in with crutches. Mind you, I can't walk on my feet because they are swollen and stuck straight down. I ask about a wheelchair they said no, you can manage. I said really, how - I can't walk on my feet! They told me I'd figure it out and made me leave like that. I could only touch just the tips of my toes on the ground lightly. I was blown away!!

We need more logic with ridiculous hypothetical parables to teach common sense and open rational thinking.

Let's suppose a young man got a job working in a place called hellhouse where he was given access to every secret place in the building.

One day as he was touring the building he pulled back the patient privacy curtain to expose the gov't sodomizing an unconscious person.

"Shame!" The media cried, "Shame on the curtain opener!" And the gov't protested, "He has exposed things we have known all along that were to remain secret. He shouldn't have done that, and he is the worst kind of criminal on earth. We shall hunt him down and prosecute him to the fullest."

And there we have the Edward Snowden vs. the NSA story to an extreme.

Incredibly, we proscecuted and jailed those for spying on the Democrats, and forced our then president under pressure to resign . . . but today it's okay if that same gov't spies on us with the premise it is "for our safety."

My friend who is attending the AHDI conference in Indianapolis just texted me that the company we are ICs for (New England Medical Transcription) was awarded the Employer of the Year.  Now isn't that strange--we're all ICs and the only "employees" are management!  They use ICs illegally and get awarded for it! Lately I have not bee doing well, short on line count, a few bad audits, I am in the dog house. I now have a fear that a I too will be let go. It is hard to work like this, wish I could just quit, but cannot right now, maybe in a few months, unless they terminate me. What if the extremely lower than ever volumes (OOW on 3 accounts at a time) is because some of the acute worktypes are being converted to ambulatory care WT since more and more patients are kept in observation status or "extended recovery" instead of being admitted. W(c)ould not some of those same-day surgery types perhaps be called ambulatory? We would likely be kept in the dark about that just like everything else, don't you reckon?
