Why do medical transcriptionists have to be paid by line.  Who started that?  Why can't they be paid per hour like other jobs?

I'd rather be paid by the line - I worked at a hospital years ago where we were hourly and I finally went to the CEO and asked him why was I getting the same pay as the girls who didn't give a darn, spent time talking, going out for cigarettes, etc and not doing much work. I was told I was before my time. Three weeks later, I had found my first paid by the line at home transcription job and I resigned from that hospital after being there 15 years. If you are paid by the line, time is money, and you try to produce more. That is my take, at least. If there's not work available, obviously that is the downside, as is the case scenario lately in this industry. That's my take, I'm sure there are others you'll see, too! :)

All night I've had 20-minute-long reports which should be mondo money makers, but they have averaged 70-80 lines each, simply because the dictators talk slow. I even put them on "chipmunk" speed but with ESL, that's not always possible.

The night before that, work ran out and I was twiddling my thumbs for several hours, forced to constantly check in for stats.

These are instances where I would like to see an hourly wage. HOWEVER, on the increasingly rare times when there are tons of "easy to plow through" work, that would obviously be to my detriment.

An incentive program in addition to an hourly base pay would be groovy. Treat those of us who kick butt and make the most for the company better than those who just float through their shift doing the bare minimum. How that would work exactly, I have no idea. Just thinking out loud is all.

Lots of places have incentive or bonus programs, both - for line pay and hourly. NM

I do have an hourly pay and incentive but it is hard to get the incentive sometimes. - Mel

I recently started using expanders (ESP) like crazy and that helps a little.

Running around socializing, not doing much work. Saw it a lot in my early days in late 70s and 80s. Had a girl that was a total social butterfly that spent most of the time on the phone and getting phone calls that I had to answer while I was trying to work because she was out flitting around when she was supposed to be working. She had a lot of boyfriends (even though she was married). Complained about it to the supervisor, was told to leave it, that she couldn't help it that she was pretty and all the men went ga-ga over her. Believe it or not.

Years ago, I had just started an in-house clinic job where they posted your previous day's line count for everyone to see. It was supposed to give us a heads up on where we were for our bonuses. After my first 3 days there, i was getting dirty looks. Supervisor pulled me aside and said i needed to slow down a bit because i was making some of the old timers look bad! I didn't slow down. I figured maybe they should spend more time at their desks and less time gossiping so they could get the incentive bonuses too.

We were paid by the hour there but the bonuses were paid on a line/minute/quality blend.

There are too many variables. When I worked in the clinic, the starting hourly wage was the same for everybody with quarterly raises. Some of us produced more than others, started on time, did not play sick to use up sick days, and did not leave a doctor stranded simply because it was break time. Some were just there to collect a paycheck. Get the picture? I do think that anybody transcribing for the worst dictators on the planet should get paid $100 an hour!!! Just kidding, but there does not really seem to be any totally equitable recompense in this business.

MTSOs started doing it when they started to get - REAAALLLY greedy. Greedier than before.

I have gone through and read some of your posts.  I see that Andrews School, MTec, and possibly CareerStep are your top notch schools.  Is there still a need for transcriptionists?  I don't want to go out and spend all this money and not be able to find a job. I am training someone and I am now reminded how far I have come, how much I have learned, and the things I take for granted and assume others know.   Basic things, just proper English, not even taking into consideration the medical terminology.  Knowing whether a sentence makes sense or not.  We really should be paid better.  But it is nice to feel smart sometimes....and you know anyone who can do this job really does have to be pretty darn intelligent.   Fello ...

I am now down to 3 companies (from 4) trying to make a living at this, wondering what I should do next.  I'd like to go get my coding diploma and move on from there as I cannot take this anymore.  Four jobs and between all of them I got $700 last week and am still waiting for checks to arrive.  I asked one of them why it wasn't DD and they said they would cut and mail the check.  Lord knows when the other one will arrive.  I don't know, don't YOU ...

I work in the path department and when a malignant case comes up that requires a CAP, I defer to the CAP summary.  The docs dictate "lymphovascular . . ." but the CAP summaries will have "lymph-vascular . . ."   The problem is today I was checking the CAP protocol for the Lung CA and the word was used both ways.  In the actual protocol itself, there was the hyphenated version as a header, i.e., but then in the explanatory notes at the end of the protocol  there' ...
