IC Invoice Template - Wondering

For my fellow IC's out there, does anyone have a template for an invoice, or a link as to how I can find one. I've usually done my own billing before, but my counting program in my other computer did all of this for me. Now I have to create one, and have no idea where to go or what to do. I use an Excel spreadsheet to keep track of my lines and every file I do.

I did my own template in a graph style with things like - IAMT

date, characters, divided by 65 and x CPL and total at the bottom. At the bottom I added a space for correspondence/notes and then very bottom "payment made to: ..., Thank you for your business - could probably add the terms of payment but this clinic cuts me a check on presentation of bill - sweet. I try to make it personal and add handwritten notes like "Hope you all have a wonderful Thanksgiving." I then save each by the date of the bill. I do fill in my figures by hand after each tape on this master but could add them on the computer I suppose. One clinic account and no real fancy stuff going on here. Sure there are other methods but this works for me. I do make 2 copies of this - one for the clinic and one for my files.

I do keep a master by month of the totals for taxes so I don't have to wait for my 1099 to start figuring taxes and for comparison, and have a section for my monthly expenses - categories of supplies, internet and phone, office space allowance, mileage, etc. and file with receipts.

Hope this helps. Did find statement templates online (use one of these for my little cleaning business) but did not include what I needed to spell out enough info for the transcription.

I have Windows 7 and tried downloading an invoice generator I found on this site, but it just created a mess on my computer! Does anyone know what link to use to get this generator? The company I work for no longer allows the MT to use their own templates. They took the templates we had, gave them to the client, and their staff either made extreme changes or threw them out and wrote new ones. Of course, the new templates never follow what the doctor says because they don't hear what is being dictated on a day-to-day basis. Does anyone know where I can find a good clean download of this program? I have tried going to the mtstars.com website to the download site & after I hit the download button & then tell it to run, it comes back and says "Error in compressed file-bad CRC"--have tried numerous times. I am a new transcriptionist working from home for a local clinic. I am getting paid 13 cents per line based on a 65 character line (including spaces) and am typing this thru Microsoft Word Office 2007. It shows the line count and character count at the bottom of the page in a box I can open up. Do I just take what the line count says it is and that is my line count for that document? Or do I need to divide the character count w/spaces by 65 for the line count? Second question.. ...

Can someone tell me how to use a template in word on consecutive pages? I know how to use it for the first page but can't figure out how to use it for say, 50 pages in a row. A doctor wants his letterhead used as a template which I have scanned in but can only seem to use it for the first page. Some of his letters are only one page and some are two or three (which wouldn't use the letterhead) but the whole dictation might be 50 or so pages. I just can't fig ...

HELLPPPP...anyone good at figuring out how to make a template? client has a form that they sent and it has headers and footers each page. They have a separate letter form, consult form and chart note. Then there is a summation sheet at the beginning and STAT sheet at the end. I was originally with one of the ambulatory groups that came through the back door of Nuance. Our old ways of doing things have slowly been phased out since Nuance acquired us. Currently I get 8.5 cpl for standard lines and 1 cpl for qualified (template) lines. I haven't gotten the email about the video yet, but I've been reading on here about the new 7 cpl rate. I get on to work this morning and found that my acct that I work on which previously did not have an templates, now has templates complete with an entire PE paragraph that I have had in my expander. I was wondering if anyone could tell me what is the easiest way to not count template lines when trying to invoice? I get a line count through Word, but I hear that Word counts blank spaces as well. I downloaded and use Flash Count, but it includes all 65 character lines, I beleive including the template headings. I hope this makes sense. Please help!! ...
