I was a patient in a local ER yesterday and had the nicest British physician.  I was directly across from central and was able to hear this physician dictate.  Clear, concise, perfect speed, a dream to transcribe.  The downside is the hospital uses Epic only - no editing by human hands (and ears) at all.  Why oh why can't us humans have dictators like this?

I've had a few dream dictators like that lately - that have the strangest foreign names

I happen to occasionally cover for a guy who is of middle-eastern descent (by name and by Googling for his picture) but who was raised in, and schooled in, the southern USA and has the most country accent EVER!

So what or who was he dictating to, then? - Was he editing himself?

Epic can be used with transcription - my health system does it.

If he was dictating, something or someone had to be doing the editing.

and its satellite clinics have all completely done away with transcription. The dictate and edit themselves. One physician dictated as I sat in the exam room, in one of the clinics.

That is what happens when they start using front-end SR ... they learn to dictate better. This hapens very quickly.

Nobody is "editing" this, either. Nobody runs along behind to clean up anything. He gets what he says as he says it. He signs it right then. It is over and done.

I once welcomed back a pathologist saying I was so glad he was here for the newbies I was training. He replied that he had learned to dictate so that he was spared having to request corrections or flag mistakes. As he said, "I learned it saved me time." He was a very nice and very smart man. In too many systems these days, I'm not sure exactly who actually reads these reports with a critical eye. They are needed for documentation for billing; if they succeed in doing that, it seems good enough

They are not just for billing. They are necessary for patient care and other purposes.

For a hospital, the reports you type are not the only ones in the record. There are also hand-written or hand-typed notes, and physicians make the same errors in them. As long as they communicate the necessary information, everything is fine. No one has ever expected physicians to be literary geniuses.

There ARE people who read those reports "with a critical eye," but they are looking for content, not spelling, grammar, and punctuation. They are "clinical documentation improvement specialists" and they are well-paid. Many of them start off in coding, because the ability to code is required.

You would be able to use your MT background for that.

Just curious if MQ is going to try to ignore yesterday's DocQscribe crash.  My CCM is on vacation so I just put an alert on my timecard.  The RTL told me that it was not an alert and deleted my note.  So downtime or not?  I guess I will just be short this week because I am not making up hours or taking PTO when their stupid system crashes and MQ refuses to own up to it.  Thats what they get for having us all upgrade at the same time.  ...

When I started out MT years back I transcribed for the local hospital ER docs group. I loved it, but moved on when work dwindled down. A friend works for the group of hospitals in management so I asked who was doing the ER transcription now, I wanted to apply. On the 3 horrible reports that I had to submit to QA yesterday because of the 3-blank rule, thus losing 3 cpl probably, QA had no more luck filling in the blanks than I and sent them through with 5-10 blanks.  That makes me so mad -- not at QA, just in general that I get penalized financially when it was the DOCTOR'S FAULT AND THE PICKY FACILITY'S "RULE."  ...
