Employment Question - Need To Know

Fifteen years ago, I worked for a certain employer and was terminated after 4 months in a particular position. I recently applied to that same employer for the same job. While completing the application, one of the questions was if I had ever been an employee of theirs, and I chose no because it had been so long. Is this going to be a problem (application didn't ask for SSN)? If so, should I try to contact the HR department and explain that I need to reapply and change my answer? Or if it gets to the point of interviewing, should I tell the person that though my answer was no, I seem to remember working for them many years ago?

I don't think that saying you don't remember working for them is very believable. Most people remember anyone they worked for, unless we're going back 35 yrs and did temp work or something.
I would probably just fess up. Tell HR you were terminated 15 years ago. Hopefully, if the termination was due to poor performance, you are now better qualified for the position. It wouldn't be right for them to assume you'd do poor performance again after 15 years.

You may want to get back in touch with the people who get the application because on every app is a blurb about your signature confirms that everything you put on the app is true and correct and if it isn't, you can be terminated.
This is something you really want to get corrected if you can.
