I wish I had words of encouragement but this has been a problem from the get-go.  Many QA people have no transcription experience and many others are just undertrained.  I have the utmost respect for QA's who really know their stuff but from what I've seen over the years, they are in the minority.  I have even seen samples that were supposedly approved by the hospitals that have horrendous errors.  If anyone has an answer, I'd like to hear it.  Management is clueless.
