Have a report with two blanks for which 2 criticals were assigned. The explanation was that the Transcriptionist either carelessly omitted the dictation, deliberately obmitted the dictation or did not understand the dictation. Bingo, I did not understand what he said---isn't that was QA is for, and why in this report its two criticals and in other reports the blanks or blands were labeled minor?
