If I proofed every single report, my paycheck would suffer a lot as

responsibility first to provide a client ready report, Im just saying that I am confident enough that I do a good enough job that I dont have to go back over what I took time in typing. Are there some errors, probably some, but nothing that would greatly affect patient care IMO.

I dont fret over whether to hyphenate follow up and I dont worry so much about missing a comma or two. Like I said, I feel I take the time as Im actually typing or editing the report that I do not need to go back over it again. I guess you could say I do my proofing as I go. It works for me and have always gotten good reviews - back when we had them.

As far as medical terms, I never, ever put a term that I have never heard before, even it is very, very clear, unless I look it up and check the meaning. I mean even its something like the bowel was resected I would never put that if it was the first time I had heard it unless I checked to see if the bowel could be resected.
