I am not the original poster, but here is an example for you. I once submitted a report with a QA marker for one word, which was left as a blank by QA. However, QA incorrectly changed another word in the report. The doctor had dictated Cantli line. I found a reference for Rex-Cantli-Serege line in Stedman's GI & GU Words book and typed Cantli line. QA changed Cantli to Cantil, which is a drug (mepenzolate) not part of the anatomy. No one is perfect, not even QA!
