going from FT to flex.....forms?

Going to flex PT 11/01. PS stated over a month ago she was taking care of the change but have never heard any more on this. Should I be receiving forms or some other verification? What about COBRA information? Don't want to bother PS unless I have to and besides she often doesn't reply and seems to receive only some of my emails. |
