We did not have a QA policy. Some of the transcriptionists were really awful; wrong terminology, wrong drugs, etc. Some things they transcribed were down right embarrasing! Yes, you are right the majority of reports are filed away mostly never to be seen again. But, there still needs to be some kind of system to strive for the best reports possible. Just in case.
