review of systems: System headings all in caps stacked at left margin, does that mean that we break them into systems even if not dictated that way?   Also, when it says to put all system headings in caps at left when dictated as such, most of my doctors will dictate some in that format, and just in a paragraph form for other parts of the exam.  Not sure what to do. TIA.
