Thanks for the response. Some of the discharge headers and subheaders are the same, but the text in the body of these headers and subheaders seems to change with every patient, even for the same doctor, so there's not much I can put into the normals (so far).
None of them seem to have long physical exams, and only a rare review of systems. (It seems like H&P's and consults had more of this. Do you agree?) I do like them, so I hope they get better. Did you find that over time with discharges, once you get to really know the doctors, they do say a lot of the same things?
