is either a PF suit, or has so little regard for anyone other than herself, that omitting dictation, altering dictation means nothing to her. Bet if something happened to her medically as a result of this, she would be the first in line to file a lawsuit.

That's what it is when all CAPS moniker is all about the Benjamins and patient safety and record accuracy means nothing.
