I know you are graciously trying to be helpful, but your post is a demonstration of a huge problem at Patient First. You are having to ask guideline questions every day. And I totally understand, because everything is so vague and confusing and totally over the top ridiculous. Having to read sentences to see if they make sense with or without pronouns, with or without excessive verbiage. We get paid on production. I understand they want a clear, concise document, but the guidelines are crazy, clear as mud and take everything way too far. It is exhausting!
