Why even use an MTSO if they are just going to outsource - to an IC

Ever since they started this major outsourcing trend to US ICs in the past few months, I've been thinking what is the point of a hospital even signing on with an MTSO??  The hospital outsources their medical records department to an MTSO who lately just outsources the same department to an independent contractor. 

Why not skip the MTSO and just hire a local independent contractor yourselves??  Get a group of different local ICs to bid on your contract.  They'll be using the same basic type of platform, no doubt for similar pay since the MTSOs have driven pay down industry wide, but you'll be supporting smaller businesses and local MTs in your area.  And you don't have to worry about the crappy quality of work you'll be getting if you wind up sent to India!

The advantage to going with an MTSO is you have a major company with lots of experience hiring well-trained MTs with regular quality checks.  You have a major company who will trouble-shoot equipment failure for you.  And you have a company who can probably design a system that will combine Epic or whatever you need with some VR or transcription.

Most ICs can and will do pretty much do all the same things with the exception of equipment failure.  The hospital can decide what to send to Epic and what to send to transcription.  And most ICs are using pretty basic platforms that any decent hospital IT department could manage.

I'm not talking about the ICs popping up who are hiring people from all over.  Many of them are behaving more like the MTSOs without benefits and a lot of them are shady.  I'm talking about local contractors you can trust, just like a decade ago.

Do you think a local IC with a large group of subcontractors could handle the work from one large hospital?  Or is that too much? 

But if the work load is doable, if all they're going to do is outsource to ICs, who even needs an MTSO anymore?
