Industry standards regarding lines - coffee

Does anyone know what the current industry standard is for getting paid per line, as far as specifics go?  I'm talking about spaces, bold, underline, as well as templates with text already in them, etc...  I've been a tranny for years and I feel like most companies I've worked for have paid for everything, even the stuff that was already in the template...but I am being told by a certain company that they pay none of that, not even spaces.  I would love your two cents.  I know the CPL rate plays a role here too, but even at 9 or 10 cpl (65 characters) I feel like it's not a good gig.
