...so we're supposed to fix them? IMO it's up to the doctor to dictate what s/he wants, and it's up to me to type what is dictated, correct information is his/her responsibility. Of course some things are clearly mistakes (like beginning of 2018, doctor dictates January 1, 2017 as DOS), but unless it's blatantly obvious am I supposed to assume that I know better than the doctor? I think not!
