I've had the same thing happen. I found 3 TRULY CRITICAL CHRONIC errors, and several not-so critical, but important enough. All the MT did was point out her an more minor errors as me nit-picking. She said nothing about her life-threatening mistakes. Oh, they kept her on as MT, costing the company a lot of money, because she was on full QA (voice against text) forever. They didn't fire her.
This was years ago. I got out of the MT business a while back, so maybe things have changed. I hope so. When a patient reads his own medical report and finds out he had a castration instead of a catheterization, he's likely to be really steamed.
