It's x3 (no space) as long as there is a numeral. This per AHDI. If no numeral, you write out "times," as in "patient has been on aspirin times several weeks."

But: 4 x 4 is used in surgery to describe that dressing, 4 x 4's for plural. And measurements are typed with spaces 3.5 x 4.0.

Lol. As I always say, this is why we get paid the big bucks (I know, I know).
