I would absolutely refuse to comply with this on the basis of patient care and personal liability.
Nuance isn't gonna pay your legal fees if your "guess" results in patient damage.
Just leave blanks or type "inaudible" in the space and let Nuance know that them shuffling the liability on to their employees so they can fake boost their "accuracy" to the client is unacceptable and a critical danger to the patients.
