Your doctors are presumably the ones paying you to transcribe their dictation. They want an apostrophe. Give them the apostrophe. No one cares about hair-splitting punctuation issues that have no effect on the life of the patient. Making an issue of something this insignificant is the sort of thing that can make one appear to be an irrational shrew. Don't be that person.
Give them the apostrophe and get on with your life.
