ORTHO SURGERY S/L FORMATIVE PATELLAR CHONDROPLASTY - sewing-bird

Here is a list of the post surgical problem and the procedure performed.  The blank space sounds like "formative" or "warmative" or something "ive".

PREOPERATIVE DIAGNOSES:   

1.  Anterior cruciate ligament tear.

2.  Lateral meniscus tear.

PROCEDURES PERFORMED:   

1.  Hamstring autograft.

2.  Anterior cruciate ligament reconstruction.

3.  Arthroscopic-assisted hamstring anterior cruciate ligament reconstruction.

4.  _____________ patella chondroplasty.
