I disagree in - Part

I concur with what you wrote except for this part:

"Even those who are more highly educated, as the ESL doctors who have come from India themselves are a perfect example of!"

Indian physicians who come to the U.S. are usually from wealthy, brahmin-caste families. They have received excellent educations at the best U.S. schools (because their families can afford it). They are usually specialists and SUBspecialists, further reflecting their elevated caste status. Ergo, their English is usually quite good.
