It was a sin for us to condense dates. It was more professional to type March 13, 1991, than 3/13/1991 or 03/13/91 or March 13th, '91, or whatever the heck version the BOS says you are to type it. We just typed it all out. The reasoning then was it created no misunderstanding as to what the doctor stated in the chart note.
