Wondering about medical plan costs at M*Modal - UnhappyMT

Just curious is anyone can tell me how much the medical plan cost is for a family or how it is structured - all visits other than preventative care is deductible then 25% for the Cigna Choice Fund HSA or the Cigna Plus.

How much does it cost for these?
