Software advice needed to become independent - atf

Hello, I have come in contact with a clinic who is not happy with their current transcription work, where do I start and what device or equipment would I need to purchase to provide my own transcription services to a team of medical doctors in a clinic environment? I have no idea as I have only ever worked for a company who provided the platform I worked off of. Thank you in advance!
