IPhones, IPads and other mobile devices ... cheap microphones ... while walking/talking, esp in/out rooms with poor acoustics, driving, etc... and downloading or uploading the dictation into Epic or other EMR ... By the time the sound reaches our ears it sounds like doc was scuba diving or in a tunnel while dictating.
