I work on the client side and had to send in a report to QA where several drugs were incorrect. The QA person replied that the client (me) filled in the wrong drugs and that the MT was correct. I promptly replied to her and to our MM rep that I have the benefit of the medical record at my fingertips and that I had indeed confirmed that what I sent back as corrections were what the physician had prescribed. So to answer your question, I don't believe anyone QA's the QA person unless it is caught by the client. They do defend you, though. Yesterday I was told that "the patient was 46 weeks' pregnant" was not an error by the MT.
