To OP: I would ask if your facility has a preference. I have one account where we just leave BPH, one voice recognition account where we leave what it expands to unless it's inconsistent with the rest of the note, and then the majority of the accounts have it in our client requirements to always expand to "benign prostatic hyperplasia" unless doctor mentions hypertrophy in the body of the note.
So basically, I'm not helpful! It looks like there are different 'rules' depending on facility.
