I just got FMLA approved for a serious personal medical issue. If you like, I can tell you what questions are asked and how much information is required by the Dept of Labor. It really is very thorough (and a little invasive, frankly). I'd prefer to fly under the radar of any government entity (not because I'm a law-breaker, ya know -- you understand what I mean).
But here's something interesting: Not only am I personally struggling with my own issue, but my mom is 96 years old, in a nursing home, and falls about 3-4 times a week. Today I asked my manager, "Well, okay, now I have up to 12 weeks' intermittent leave approved specifically for my health issue/followup appointments/whatever. I'm certain I won't need all of that time (and my pay will initially be taken from my PTO anyway; after that's depleted, I won't be paid at all), but what if my mom needed me after a fall? My manager said that you can't use FMLA for mixed purposes, or anything other than the reason(s) specified in the doctor's request. She also said you can't simultaneously request 2 concurrent FMLAs. I would guess that this can't be strictly monitored, but of course my employer is expecting that I'll be honest and forthright with them, which I will.
