You'll have to ask your employer how they want headings to be transcribed. If a dictator says present illness, we typically expand it to history of present illness. We have a list of approved headings to use. So, if in doubt ask your company how they want it done. I assume you are joking with the rest of your question, and I don't have an answer for you.
