I thought that you probably had not seen how corrections work from the medical record side. We can't make corrections unless reports are flagged, when they're gone it's too late and the providers don't review their reports as well as they should so things get missed. I don't know if all facilities are like that but it's how we are set up. When patients request corrections to their chart, the providers have to do an addendum instead of changing the note. When I worked as an MT I dreaded my audits, I think it's amazing and awesome that this is your first one in all these years.
