My EMR had me down as DNR status!!! As a 50-something in VERY good health, needless to say I was VERY upset about this!
When I expressed my concerns, of course they laughed it off -- pooh-poohed it even. "Oh, it's so easy to make a mistake sliding...." They mailed me hard copies of everything to assure me that it had been changed.
Needless to say, I changed doctors. BRING BACK PAPER CHARTS! No "sliding" errors with those!!!
