I did QA from 2002 to 2008 and got paid $15, progressing to $16 an hour for 2 large companies. We did transcribe when needed on new accounts or accounts that were behind for the same rate. I was offered, but never took a couple jobs where they paid by the line rate, so can't comment there. There are still companies out there that pay by the hour, although the last I heard from their ads on mtjobs.com, the rates were more down around $11-$12 an hour, sadly.
