I used Bayscribe for straight typing.
I made my own templates and abbreviations. If I was lucky I would get doctors that said the same thing in the physical exam, so I made short cuts to those. Most have information at the top that you filled in.
I worked for a large company and I made good money for 14 years till they split the company and then I just decided to retire.
