If traceable PHI is appearing on documents, that is the fault of the facility not caring and the MTSO who contracts the work not doing its job. I've seen this discussion many times, but offshore contractors are required to be HIPAA compliant, if you'd ever read a standard contract you would know that is true. Companies are supposed to send and receive information over secure servers, PHI is supposed to be encrypted. Identity theft occurs domestically as well as off shore and it sounds as if the scenario you describe needs to be addressed at the facility and MTSO level, perhaps you should bring it to your employer's attention.
