It is per 2-year cycle. Keep in mind that you do not have to maintain an AHIMA membership with their certifications. There is a renewal charge that they will assess if you don't have a membership, but it is quite a bit less than the membership.
Yes, their website is very confusing. Don't think you are alone with that complaint! They also improve it regularly to make it even more challenging, like a kind of randomly shifting maze puzzle.
