No single input record is going to be 99% or even 98% correct. Doctors at that hospital are either wasting their high-dollar time on correcting mistakes OR those mistakes are not getting corrected. Either way, the hospital bean counters are shooting themselves in the foot. It may just be that their dinosaur brains haven't figured it out yet.
