Documentaries about entire towns are more appealing. Most folks don't even know what a medical transcriptionist is or does, or they've never even heard the term.

In addition, the reality is that just about all big corporations have the wage/salary disparity you cite, with fat cats at the top and slaves at the bottom. It's not unusual this day and age, so it's "unremarkable." Huge corporations that are household names (GM, IBM, etc.) would get attention so may be more desirable subjects of a documentary. Case in point: Roger and Me by Roger Moore.
