I always put the patient age at time of appointment. It just made sense to me. Now I am going to be wondering.

I always assumed, no matter when the dictator is dictating the report, that the report is based on the DAY HE SAW THE PATIENT, and therefore the patient age would be the age at the date the doctor saw the patient. Of course, if he is dictating a current note, like a telephone note, and states something like, "I saw her in January but now, in July, she calls me and states she is failing" then the patient age would be her current age. AUGGGGGG !! Just one more thing open to interpretation and get dinged for right?
