I am hoping someone can help me with where to start or how to go about coming up with my own contract to submit to a local hospital. I have only ever worked MTSO work, so never had to do any of this for myself. It's not a complete hospital contract, it's a single physician, but they want me to submit my contract and my cpl requirements to them. Help please. TIA.
