Whatever your process is for warnings and firing should be started immediately if not already started. You need to document the insubordination and let her know it will not be tolerated. You should not have to put up with abuse from an employee like that. It is not fair to you, the other MTs in the department, the doctors, the patients, and it is your responsibility to protect them when stuff like this happens and put a stop to it. I do not envy your position, but you have to do your job and get her decently compliant with her duties and exhibiting professional behavior or fire her. She might quit if you start the documentation and correction process. Good luck!!
