I need some help PLEASE.  I keep getting flagged when I expand the ORIF abbreviation, no matter which way I attempt it. 

open reduction internal fixation

open reduction, internal fixation

open reduction and internal fixation

Are these correct or incorrect?  Are there others I am not aware of.

The Stedman's Abbreviations, Acronyms & Symbols (Third Edition) shows with a connecting "and" as "open reduction and internal fixation." 

The Stedman's Orthopaedic & Rehab Words (Third Edition) shows with an inserted comma as "open reduction, internal fixation."

On internet search, it is simply "open reduction internal fixation." This was the way I had transcribed it for years without any problems. 

My Stedman books are older editions, so maybe a newer edition narrows it down to one way.
