It depends on the accounts. There are weeks where I have to do accounts/specialties I don't know how to do, and the time it takes to carefully go through them and searching through old notes make it impossible. Of course, we are told to sign out if we have to research, but if I'm actively looking for something I'm working. I'm told I have the wrong mindset on this, so maybe I do. It is not a guarantee, but you definitely don't want on the report when you don't make it.
